It certainly looks like a siren. It appears that you are right about it being an ex-police bike.

I think the Moto Guzzi police bikes in the USA also had friction-drive sirens.

I had a quick furtle in the web, and it seems that friction-drive sirens were regularly fitted to US police and military bikes from the 1920s on.
You can still get them if you want one!
